Start Menu> Run > Type "regedit":
Expand: HKEY_CURRENT_USER\Control Panel\
and remove the folder mouse
then put this default mouse settings and restart (this ill recreate new folder)

format is normally the last thing you do but yes it may help if it is not a hardware issue --motherboard or USB port(s). Other than that the issue your discribing sounds like the issue I have if I'm using d3d9 with di off. Also when you try other players inis you really wanna make sure in the user.ini NEXGEN ID is deleted or the whole portion of the nexgen line in the ini so it can redo ur ID.
Well formatting didn't work so I'm ruling out a hardware problem. I'm on a budget build anyways so I wasn't really expecting top of the line. Thanks for the help everyone.
Maybe you're just low on fps?
What if you halve your dpi, does the issue still persist?

It's weird. Every other game I play the mouse works fine except for UT.
Before I never had a problem until a half a year ago. I didn't change any of my settings, download any video drivers that would affect it or anything.
I shouldn't be low on FPS, the game is over a decade old now.
switch to 500hz, make sure nothing else on that usb root controller if possible, keep your direct input off, switch your mouse dpi somewhere in the range of 400-800 if possible, then, the biggy thing to try if you haven't tried it yet, is to download a utility called RInput. It captures your mouse's raw input and injects it into UT, allegedly bypassing all the Microsoft mouse setting crap.
Nope no luck. Switched from 125, 500, then 1000hz. My mouse DPI can only go from 450/1150/2300. Al though, I ran an avast scan before POST and it found 12 infected files, and one of those couldn't be removed. I might have to refomat everything again.
