There isn't a permanent fix....I've just seen logs and wondered if you knew how to fix the issue. It happens when the map gets corrupted and the md5 hash of the map changes. It stops people from changing client side map and the server allowing it as it could be cheats...

delete all your maps and redownload them one by one.

just move the player back and disallow his teamswitching abilities dont even bother warning him because 10/10 times the person tries to go again and realises there not switching at all then they start spamming it thinking it will work and still there dumbfounded which is then they realise it wont work and there not allowed to do it
